Here at Rayville Recovery, we have a dedicated staff position of ‘Discharge Planner’. This team member works solely on the coordination of resources and making those resources available to clients as they complete treatment at the inpatient rehab facility.
We begin comprehensive discharge planning at the time of admission, and coordinate follow-up medical, mental health, aftercare/IOP, Sober-Living placement, and coordination with legal entities such as judges, family courts, drug courts, and parole/probation departments.
Every client is recommended to relocate to a sober living home for no less than 90 days post-treatment. After treatment, each client will be given client-specific documents that include a ‘SNAP’ analysis as well as a schedule of reminders that outlines any scheduled or recommended appointments for mental and physical health services as well as AA/NA/AL-ANON meeting times.
